The New York State Department of State’s Office of Fire Prevention and Control’s Training Authorization Letter (.pdf), is to be used as the registration for all County and New York State training courses that are held here in the County.

Registrations, using this form, will only be accepted from the Chief of the Department or their authorized designee(s).

Registrations must include:

  • Course name
  • Member’s name
  • A telephone number to allow contact with the member to provide course scheduling updates

In addition to being used as a registration form, the form is also an acknowledgement based on the signature applied that:

  • The Chief (or their official designee) has approved the member to attend the specified class;

  • the Chief acknowledges that the student has medical clearance to use Self Contained Breathing Apparatus (SCBA), and is authorized within their home department to use SCBA and participate in interior/exterior firefighting evolutions;

  • the Chief acknowledges that the member has completed all prerequisites for the course;

  • the member understands the importance of safety during the training course and acknowledges that if the instructor believes the student’s behavior or abilities may cause a safety risk to them self or another student, the instructor has the authority to remove the student from the class, and

  • if the member is under the age of 18 years, their legal guardian has read and agrees to the behavior and safety standards of the class.

The completed form can either be mailed or faxed (845-486-3998) to our office.

As registrations are accepted, the student’s name will be added to the course list and provided to the SFI or CFI for the class.  A list of scheduled classes and students enrolled in each can be found on this site under the Training Schedule tab.  We are working on a new web site to post upcoming County and State training courses, as well as allowing you to register for the courses online.
