How long has the agency (or individual) been operating in the community? Is the agency licensed or accredited by a governmental agency or professional association? Can the agency provide references from clients?
How does the agency train its employees? Does the agency carry malpractice insurance? Does it insure employees who provide financial services or who do banking and shopping for clients?
By whom and how often are home care workers supervised? Who do you call with questions or complaints about services? On what grounds can you request a change in workers? Does the agency provide substitutes? How will an emergency request for assistance be handled? Are workers employed by the agency or sub-contracted from another agency?
If you need personal care or therapy, will a professional perform an assessment in your home? Will a plan of care be written that you can share with your family, your physician, etc.?
If Medicare is your payment source, be certain that the agency is certified to bill Medicare. Make sure you understand exactly what the services will cost and what you will be receiving for your dollar. Request an hourly rate as well as a weekly total.
Does the agency provide written statements describing services and their costs? Is there a charge for telephone consultations? Is there a fee for the first consultation?
Are there minimum hour or day requirements? Are the services billed only by whole hours? By half hours?
Will there be a fee for a new assessment as new problems occur? Is there a set fee for everyone or do they use a sliding scale to accommodate lower income clients? Are there additional charges you must pay? How will you or your insurance company be billed? How are late payments handled?
