  1. Our new online application can be found online. The new online portal will be available starting April 21st, 2025. If you have not done so already, please log in and create an account. For existing Children’s Camps, please use the search bar to find your camp. For new Children’s Camps, please create a new facility. Please complete this process by April 30th of the current camp year. Please note paper applications will no longer be accepted.
     
  2. Camp Safety Plan Statement (DC HD-90)  Any and all changes to your plan must be submitted to DCDOH 30 days prior to camp orientation for review and approval. A Safety Plan Template is available for your convenience.
     
  3. Camp Written Plan Checklist (NYS DOH-2040)

    Please note: Items 6 and 7 that follow must be sent directly to the Dutchess County Department of Health. Ignore any contrary instructions that may appear on the forms -- send them directly to Dutchess County. Failure to do so will delay your approval to operate.
     
  6. State Central Register Database Check Form (NYS LDSS-3370) - Must be completed by the Camp Director using the line-by-line instructions. 
     
  7. Prospective Children’s Camp Director Certified Statement Form (NYS DOH-2271) - Must be completed by the Camp Director.
