Community Health Assessment (CHA) & Community Health Improvement Plan (CHIP)

Every six years, New York State publishes an updated Prevention Agenda (PA) aimed at improving the health of New Yorkers. The PA identifies priority areas that local health departments use to create their Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). 

Community Health Assessment (CHA)

The Dutchess County Department of Health puts together the Dutchess County CHA including information from:

  1. A community health survey where residents are asked questions about what health challenges people face in their communities
  2. Analysis of data from other sources, like the New York State Department of Health and the Centers for Disease Control and Prevention

The CHA guides the work and necessary actions of the Department of Health and its partners to create the Community Health Improvement Plan (CHIP).

Community Health Improvement Plan (CHIP)

Using the information learned from the CHA, the Dutchess County Department of Health, in collaboration with community organizations and other stakeholders, creates the Community Health Improvement Plan (CHIP). The CHIP selects priorities from the New York State Prevention Agenda to be focused on for the next six years. The CHIP includes specific interventions and activities aimed at improving the health of Dutchess County residents. 

The Department of Health and its partners are hard at work on the new Community Health Assessment and will finalize it by the end of 2025.  This will then help select priorities for the Community Health Improvement Plan that will be in effect through 2030.
